On [REDACTED] [REDACTED] was walking back from the [REDACTED] and as he took the first step going down towards [REDACTED] his foot got caught in the carpet and tripped over the next 2 steps and resulted in falling back first and hit the middle step with his back. [REDACTED] got back up and felt pain in his left shoulder and his lower back. [REDACTED] reported it to the MOD and also to security for reporting purposes. He was offered EMS but [REDACTED] declined and finished the rest of the shift
