Before the incident

Team member was in the process of mounting a plate to a sleeve using the semi-automated mounter. The plate he was using had curled edges and wouldn 't lay flat.

What happened

Team member put a rubber hammer on top of the curled edge to help it lay flat. As it was being pulled into the machine he reached over the light curtain and attempted to remove the rubber hammer. His finger got stuck on the adhesive tape and the lay-on roller came down on his finger.
