On [REDACTED] at approximately [REDACTED] at the [REDACTED] IN site a Mill Operator was removing the flat head set screw from a rubber mill knife The screwdriver slipped and caused the Team Member to become frustrated He then purposely struck the toolbox with his right hand The deliberate contact with the toolbox resulted in a laceration to the right index finger (base knuckle) The Team Member received local 1st aid but needed further medical attention resulting in three stiches The Team Member returned to work the same day This is an OSHA recordable not a [REDACTED] recordable incident
