Working in a ceiling grid from a 6ft A-frame ladder

TM was working from an 6ft A-frame ladder from the fourth rung to access a work area in a drop ceiling grid above a toilet in the [REDACTED] employee bathroom. As he was working the TM states that he was repositioning in anticipation for a tool to be handed to him and states that the ladder slipped on the tile causing him to lose balance and fall to the toilet. This resulted in pain on his left side . The team member stated he was okay at first but the pain didn 't go away and he had shortness of breath so he panicked and had his girlfriend come pick him up from the warehouse to transport him to an emergency room. We were unable to get into contact with the TM once the supervisor learned of the incident and were notified of the ER care and treatment the next day. The TM did not contact the supervisor to advise of the incident and the supervisor only became aware after a work order coordinator called him to make him aware.
