Manually repositioning straightening the reel arm to address misalignment and capacity-reached stoppages on the reel-up.

The operator was troubleshooting Foam Line 11. While adjusting the reel arm on the reel-up machine an improperly grounded wire allowed electrical current to energize the machine’s frame. When the operator placed his right hand on the reel arm and rested the other hand on the reel-up the circuit completed and he received an electric shock.

Electrical shock to right hand with transient symptoms (pain tingling); employee later reported chest discomfort and was evaluated (EKG normal) out of an abundance of caution.

Electrical current leaking from the reel-up machine due to a loose neutral missing frame-to-ground bond and a water-damaged dancer-arm circuit board.

Electrical shock to right hand with transient symptoms (pain tingling); employee later reported chest discomfort and was evaluated (EKG normal) out of an abundance of caution.
