Working on Baler vacuum motor on rooftop
What happened
At approximately [REDACTED] on [REDACTED] with slight rain conditions [REDACTED] Maintenance Supervisor [REDACTED] [REDACTED] and [REDACTED] Plant Engineer were on the roof of the plant performing repairs to the Vacuum blower motor belts to the Baler room. Equipment had several issues over the weekend and the belts were being replaced to get baler operational. Equipment was locked out and work was performed. Upon the work being complete and the motor was re-energized it was immediately noticed that one of the belts had rolled over and twisted. Equipment was powered back down and locked out. [REDACTED] began to hand roll the belt with his hand to check the alignment and [REDACTED] reached in to assist in rolling the belt as it can become difficult to roll by one person on at which time as the belt was being rolled by [REDACTED] and [REDACTED] assisting. [REDACTED] 's finger was caught underneath the belt as the roll was occurring. [REDACTED]’s middle finger was injured at that time an
