Before the incident

At approximately [REDACTED] on [REDACTED] a shipping employee drove from the warehouse to the north side of the [REDACTED] to deposit some dunnage (paper waste) for disposal in the slusher as is standard practice The employee was driving a [REDACTED] forklift that was being used to move the dunnage The employee on the [REDACTED] then drove out of the furthest south rollup vehicle door on the west side of the [REDACTED] basement and made a lefthand turn to begin traveling south back to the warehouse At the same time another employee was exiting the [REDACTED] basement via the pedestrian doorway on the east side of the [REDACTED] 6th dryer section heading into the center aisle The pedestrian stepped into the center aisle via the doorway had the door opened which obstructed the line of sight between him and the forklift driver momentarily As the pedestrian stepped beyond the doorway into the center aisle his foot was struck by the [REDACTED] as it traveled past him The weight of the vehic
