There were two 56675 rolls that were run together due to winder shift at top of roll Roll log was placed behind winder 3rd Hand placed splitting wedge between rolls and tapped wedge down approximately 1 The 3rd Hand then used the 24 sledge to drive the wedge into runtogether rolls to split them apart The sledgehammer face did not hit the wedge squarely and the wedge flew out and struck an observing Employee watching the procedure on the nose cheek The Employee that was struck was standing 6 away 90 degrees from the wedge location The Employee received lacerations on nose bridge and cheek Employees were wearing all proper PPE Employee was treated by ERT members and transported offsite to medical facility
