Before the incident
PM2 was on a coater break EE put up the barricade tape on the front of the coater catwalk to prevent access and actuated the lever to lower the blow off box The employee then lowered the barricade tape and entered the coater catwalk The EE then proceeded to the back side of the catwalk and reached to remove hay from the machine While they were doing this the blow off box which did not lower when the lever was actuated dropped and landed on top of the EEs right foot impacting just above the steel toe of their boot
