Operating the outfeed end of the auto stacker

Incident happened on the Dry auto stacker. [REDACTED] was operating on the outfeed end on course make up when he had a skewed board at the top of the auto stacker around the Wonder wheel. He walked up and leaned over to try to fix the skew but could not get to the board and then decided to jump out on top of the chains to grab the board and fix the skew. During this time [REDACTED] did not lock out the machine prior to jumping out onto the chains. [REDACTED] was operating the infeed end of the auto stacker during this time. He was unaware that [REDACTED] had gone up to fix a skewed board and right after [REDACTED] stapled the tag onto the last board of the current pack they were running he turned the machine back on from his panel. [REDACTED] was standing on a board at the top of the auto stacker chains at the time the chains were started back up and rode the board down towards the decline chains and falling onto the tray sorter deck.
