Walking down a ladder

At approx. [REDACTED] on [REDACTED] during a routine preventative maintenance on the second floor of the decomposer 1st Class Tradesman [REDACTED] lost his footing while scaling down a fixed industrial ladder. [REDACTED] fell approximately 4 feet and sustained an injury to his right shoulder. [REDACTED] radioed 1st Class Operator [REDACTED] to come to the second floor. 1st Class Operator [REDACTED] was the first person to find [REDACTED] on the floor. He yelled in the stairwell and [REDACTED] came down to assist. [REDACTED] remained with [REDACTED] while [REDACTED] went to alert Site Manager [REDACTED]. When [REDACTED] arrived at the second floor [REDACTED] reported he was unable to move his arm and was in a lot of pain. [REDACTED] called [REDACTED] at [REDACTED]. Paramedics arrived at [REDACTED] and took the patient to the local emergency center for evaluation shortly after.
