Before the incident
Trying to get a spyder fixture that was stacked under two front mass fixtures. As he slid pulled the spider fixture out the front mass fixture that was on top of it dropped fell and the top support beam of the fixture landed on employee 's head.
What happened
The front mass fixtures (steel cart weighing approximately 55-65 lbs) fell from a height of about 2 feet onto his head leaving a 3-4 inch laceration across the back top portion of his head that required 5 staples to suture. Employee 's head was tilted in a slightly downward position upon impact.
