What happened
While spotting the forklift the employee had positioned himself beside of an Instrument Spool sitting upright on the ground set upon its flange as the employee spotted the forklift the assembly tipped over and the 7 10K frac valve landed on the employee 's left Foot. Note the assembly had been sat down on its flange where uneven (and wet) ground conditions caused the assembly to become unstable and spontaneously tip over with no contact from the employee nor the forklift.
