EE ALLEGES APPROXIMATELY [REDACTED] SHE WAS DRIVING HER FORKLIFT WHEN SHE STARTED TO FEEL NAUSEATED THEN AFTER DRIVING FOR A BIT SHE STARTED FEELING INTENSE LIGHTHEADEDNESS AND THE FEELING OF LIKE SHE WAS GOING TO PASS OUT. EE DENIES OTHER INJURIES AT THIS TIME. EE OFFERS SHE PULLED OVER AND SAT DOWN WHEN SHE HAD A BRIEF MOMENT OF LOSING CONSCIOUSNESS. MEDCOR RN RECOMMENDED EE CALL 911 D T EE STATING SHE COMPLETELY LOST CONSCIOUSNESS. CALL WAS INITIATED BY SECOND SHIFT SUPERVISOR [REDACTED]. EE DOES NOT HAVE THE PHONE NUMBER OF HER WITNESS THE FIELD WAS LEFT BLANK INTENTIONALLY ON THE REPORT.
