EE was clearing a jam but failed interlock failed.

What happened

EE reported that there was a paper jam EE opened the side cover to the equipment to clear a jam EE states that the machine should of stopped when interlock door was opened as this is a task done daily to align or remove paper jams after the investigation this machine the interlock is damaged and failed EE 's glove was grabbed when the equipment activating pulling and crushing her left hand in the conveyor [REDACTED] had some bleeding and some pinches on EE 's finger. EE was sent to Exer for a medical evaluation.
