Before the incident
The employee was selecting a [REDACTED] pallet jack for use at the start of the shift. The worker was attempting to climb over a parked pallet jack and move between two parked units in order to access the equipment.
What happened
While moving between the parked pallet jacks the employee inadvertently contacted the control handle of one unit. This unintentional activation caused the pallet jack to move forward trapping the worker’s foot between the moving pallet jack and the stationary unit positioned in front of it.
