What happened
On [REDACTED] at approximately [REDACTED] [REDACTED] was returning to their vehicle while finishing up a delivery on the [REDACTED]([REDACTED] [REDACTED]). [REDACTED] was not looking straight ahead of them instead looking at their phone when his head came in contact with the concrete metal stairwell leading upstairs. This caused a 2-inch laceration to the top of [REDACTED] 's head. Called dispatch and was advised to return to the lot so that he could seek medical attention. [REDACTED] went to MedExpress and was advised to make a follow-up appointment on [REDACTED] to check for concussion. [REDACTED] ended up getting staples to connect the cut and the follow-up appointment advised that he did not receive a concussion. [REDACTED] was not given any work restrictions.
