Employee was moving pallets of chemicals and speaking with another co-worker.

What happened

On [REDACTED] at approximately [REDACTED] the injured employee received a crushing injury to their right great toe. Employee states that they were moving pallets of chemicals with the electric pallet jack. While they were standing at the handles pulling towards themselves to reposition the pallet the electric pallet jack pushed up against their boot forcing their right great toe upward hyperextending it. There was no bruising after about 30 minutes but by the end of their shift there was bruising. Employee signed the declination form stating that did not want to be seen at that time. Employee used an ice pack for the swelling. [REDACTED] employee states that the pain is still there and requested to be seen. Employee was seen and treated at Mercy Urgent Care were it was diagnosed that they had fractured their right great toe. Employee was prescribed to wear a walking boot and to be off from work until seen by Orthopedic to be reevaluated. Employee was wearing all proper PPE at the time
