employee was pulling product from the freezer.

At approximately [REDACTED] on [REDACTED] employee received a smashing injury to their left foot on the side right before the steel toe part of the boot begins. Employee states that they were pulling their pallet from the bottom rack in the freezer. The pallet jack was then stuck within the pallet. Once the pallet jack gained traction it came out quick and smashed their left foot pinching it between the rail and the pallet jack. Employee was wearing proper steel toe leather boots at the time of the incident. Reporting an 8 10 on the pain scale. Employee was instructed to use an in pack and place on the injury for between 30-45 minutes. Employee does not want to seek further medical attention and signed the declination form stating so. Employee was instructed to contact their supervisor [REDACTED] in the event the pain worsens and they would like to be seen.
