Employee was walking in front of the pallet jack and slipped on snow in the freezer.
What happened
At approximately [REDACTED] on [REDACTED] employee slipped on snow in the freezer. Injured employee was using an electric pallet jack to pull product from the freezer. They were walking in front of the pallet jack and slipped on the snow falling forward hitting their face first. There was no concussion just a laceration to the upper lip and a loose tooth. At the time of the incident employee states that they were at a 6 10pain. The floor in the freezer is concrete and of course with it being below 40 degrees it makes it hard to walk. Employee was wearing proper shoe protection that were only one month old.
