Operating a lift putting a pallet in the overhead rack

While placing the pallet in the designated location the operator did not notice a loose case on top. As the pallet was positioned the top rail pushed the loose case off causing it to fall onto the top of the forklift cage and break the bottles inside.

The associate was taking a pallet from the dry wrap area to location DA 343C. She began to deposit the pallet in the aforementioned location. During the deposit there was a single case sitting on top (A1 sauce glass case) this case was on top of the other cases of that pallet. The associate then made contact with the upper level of the rack causing the single glass case to fall and make contact with the overhead guard of her forklift. The glass in the case was broken and some of the pieces of glass struck the associate.
