What happened
EE is having wrist pain from assembling units and using tools. EE cannot pinpoint when pain started. EE left for work [REDACTED] never mention anything to supervisor about pain. [REDACTED] EE went to ER for the pain. [REDACTED] EE came into work only to notify HR he went to Dr and go through EEs benefits and discovered EE does not have Insurance through Trystar. Dr. [REDACTED] did not state it was work related or assigned any work restrictions. Dr. [REDACTED] did excuse EE from work [REDACTED] - [REDACTED]. EE had XR on wrist results show no findings (see last page of attachments in Dr Summary Visit.) EE has been instructed by Dr to take Tylenol ibuprofen and wear a wrist guard.
