What happened
A statement or Injury Report have not yet been received from [REDACTED] after numerous attempts. He came into the office on [REDACTED] and provided the After Visit Summary from the ER and a RTW note. According to the Summary he sprained his right ankle.I asked him if he was skipping steps as was going down the steps. He said no. The steps are narrow and his foot slipped of the step causing him to fall down the steps. He said he didn 't really think anything of it until he woke up the next day and his ankle was swollen.
