Getting ready to load the press
What happened
On [REDACTED] at approximately [REDACTED] an incident occurred in our Flat Press Department. The incident occurred shortly after the injured associate was instructed to load Flat Press 1 before going on break. The injured associate began the loading process and reached the final stage. The associate stated that while grabbing the handle on the monorail to pull-turn the loaders out of the press (at the pond end) his right hand accidentally made contact with the pond railing or shield designed to prevent falls. The injured associate was unaware that the monorail could extend back as far as it did which could have potentially led to the incident.
