What happened
The patient was climbing out of bed. I went over to assist him and he began to get agitated. I then went to hit the call bell for help. As I grabbed and pushed the call bell he lunged at me. He scratched my chin and got a hold of my collar. He broke my necklace and refused to let go. At that time no one had responded so I began to yell for help. The patient had a tele sitter camera watching them. Throughout the entire incident they never sounded the alarm.
