Employee was preparing to start the inspection process on a pallet that contained 25 cartons (5 layers 5 high) of apparel. They used a box knife to cut the shrink wrap that was wrapped around the cartons on the pallet. They placed the box knife on their workstation and started to remove the wrap from the pallet.

As the employee was removing the shrink wrap the wrap got stuck on the bottom carton. The employee bent down to remove the wrap from where it was stuck and didn 't see that the 4 cartons on top of this carton became unstable and started to fall. The cartons fell on her and knocked her to the concrete floor. The employee stated that her back neck arm knees legs

Heavy cartons of apparel stacked on top of each other caused a decrease in the structural integrity of the bottom carton which caused the cartons to lean when the shrink wrap was cut and fall on the employee.
