Employee was placing an IV in a patient and had removed the needle with the safety device engaged
What happened
After IV placement the employee removed the needle and engaged the safety mechanism While removing gloves the left glove became caught on the end of the needle being held in the right hand This action disengaged the safety device causing the needle to rebound and puncture the employee s left palm
