EE was working on the warehouse floor in front of door 02. EE was preparing to unload and receive a trailer and was waiting on the trailer to back into door 02. Because it was dark outside the EE could not see through the dock door window so approached the dock door window to look through.

To access the dock door window the EE had to step down from the warehouse floor to the dock door floor which is an elevation change of about 18 inches. The EE used both hands to brace for the step down but did not notice a broken chunk of pallet wood on the floor. When the EE 's foot landed on the wood chunk the wood rolled and caused the EE 's ankle to also roll and sprain from the movement.

EE 's right ankle was rolled and sprained. As EE fell their left arm scraped against the nearby dock machinery causing a small bruise on the back of the left upper arm.

When the sprain happened the EE fell to the floor on their side. The EE was moaning in pain. The wood block was the catalyst but the injury happened as a result of the rolling of the ankle when the wood rolled underfoot.
