What happened
Two employees were tasked to remove a bleeder blind from a 10 diameter steam line. A Safe Work Permit was accepted; zero energy was confirmed at the bleeder blind and a nearby highpoint vent valve. Employees broke the seal of the bleeder blind and removed all but four bolts to hold the blind before removal. No leakby was noted during this process. While the employees were getting prepared to completely remove the blind from the four bolts steamcondensate came from the line deflected off the partially opened blind and contacted three employees two employees removing the blind and another employee on a lower level.
