What happened
A journeyman pipefitter and an apprentice were engaged in the installment of a 30inch by 2 3 58 pipe spool weighing approximately 1000 pounds on the fin fan structure. The spool was situated between a 30 butterfly valve on the west flange and a 30 check valve on the east flange. The task required realignment which necessitated loosening the 1 38inch bolt studs and 2 34inch nuts on the east side to facilitate the slight rolling of the spool and bolt installation on the west side. During this operation the workers were originally using a combination wrench to loosen the nuts when they encountered a nut that was bolt bound due to the pipes weight. As the journeyman was positioning the hammer wrench onto the nut the apprentice delivered a striking force with a maul based on an unverified assumption that the tool was fully seated and the journeyman was ready. As the hammer wrench was struck the journeyman 's right fifth digit finger was compressed between the hammer wrench and the nut. The
