What happened
At approximately [REDACTED] on [REDACTED] an employee was vacuuming out coke in the C quench tower located in the VCM unit. The vacuum hose was inserted into the manway of the tower and the employee stated that he was using an openended utility hose to spray water inside the manway to help break up the coke. While doing this the vacuum hose unexpectedly failed causing the hose to shift aggressively. The vacuum hose contacted the employee on the mouthlower jaw causing an injury.
