What happened
Direct Causes Underlying Causes Thoroughly Explain Left foot got caught under the pack. Body kept moving but the foot did not. The poly board was too close and caused a tight area. Didnt use same size boards on scrap could not run the scrap out any further do the board hitting the gate. Housekeeping. If the boards were not left on the floor from the previous run there would have been less congestion and would have helped eliminate a trip hazard. Tring to save time and effort.
