Root Causes 1. Associate walking backwards through isle way measuring 44 inches wide and desk measuring 34 inches wide was not aware of his surroundings and the potential pinch point between the carpet basket and the desk he was moving. Turning the desk sideways 30 inches wide would have allowed more room to maneuver the desk through the isle way. 2.Associate was not aware of his surroundings at all times by taking 3 steps back keeping 2 hands clear and doing 1 doublecheck to ensure he was working in a safe non congested area before attempting to move the desk.Corrective actions 1.Coached associate on ensuring a clear path is identified before moving equipment.
