What happened
Root CauseThe incident occurred as a result of an associate handling a personal pocket knife in an unsafe manner within the work area. The knife was handed to another associate while in the open position. At the time of the incident the individuals involved were engaged in nonwork taskrelated interaction. Additionally the use of a personal item not authorized for workplace use bypassed established approved safety tools.Corrective Actions1.Safety Standdown was held with all associates to cover workplace conduct and reminded the associates any use of personal knives is prohibited and only approved PSI issued knives are allowed. Completed 6 10 20252.Leaders were instructed to increase their presence on the floor with a focus on proactive coaching and realtime observation of associate behaviors and reinforce a culture of safety. Completed 6 10 20253.Following a thorough investigation of the incident it was determined that two associates had violated established safety and conduct policies.
