What happened
During a routine Clean-In-Place (CIP) operation the Control Room Operator (CRO) requested maintenance to empty IBCs 7 3 and 8. The Maintenance Mechanic (MM) arrived on the 3rd floor and began work. While disengaging the valve brake—installed incorrectly on the T-cart side—the cart moved unexpectedly and struck the MM’s leg.
