Before the incident
EE was doing an inspection of the Payday lift return terminal He noticed an oil puddle on the bullwheel He climbed down under the floor to inspect the cause of the oil and clean the spot When under the floor the terminal only has a soffting skin which cannot support a persons weight There are narrow support beams and sheets of plywood we move around to support ourselves while working in that space According to the EE he had finished his inspection and cleaning and was moving to crawl out from under that space He said that he must have shifted his weight wrong and accidentally put his weight on the soffiting The soffiting gave way and he fell 1215 feet onto the ground EE landed feet first and then onto his left hip EE complained of pain in his left hip leg pelvis region When I arrived on scene EE was standing having been helped to his feet by team members EE quickly felt uncomfortable and needed to sit back down on the ground I spoke to him about medical attention and EE felt he could w
