[REDACTED] had purchased some mulch and was loading it into the back of his truck to get it out of the way- so they could finish cleaning the [REDACTED] stuff up.
What happened
Employee was standing on the tailgate of his truck with the pallet of mulch lifted on the forklift so he could load it. He went to take a step back and didn 't realize how close to the end of the tailgate he was and fell backwards. He landed on his back and hit his head on the grount.
