Before the incident
EE was investigating an ink leak on Bag Machine 6 's upper front printing press ink pan. EE blindly reached to the back of the right underside of the ink pan near the anilox cylinder and print cylinder shaft journal to wipe the ink off of the underside of the ink pan with a rag using their left hand. As they were wiping the ink the rag and left index middle and ring fingers were pulled between the anilox cylinder and print cylinder.
What happened
EE was investigating an ink leak on Bag Machine 6 's upper front printing press ink pan. EE blindly reached to the back of the right underside of the ink pan near the anilox cylinder and print cylinder shaft journal to wipe the ink off of the underside of the ink pan with a rag using their left hand. As they were wiping the ink the rag and left index middle and ring fingers were pulled between the anilox cylinder and print cylinder causing a fracture and avulsion to the left index middle and ring fingers.
