Before the incident
EE was conducting a film roll cut over on [REDACTED]. The Extruder right air cylinder was not working. EE proceeded to ask another employee for help handling the control box while EE pushed the right fork down utilizing a piece of metal core approximately 1 ' 1 2 long and a diameter of 3 to be able to release the roll from the fork. The fork moved back to its original position while EE was holding the shaft.
