Employee was pulling on the super sac

Utilizing a chain hoist on the 5th floor of the SPC I Tower the employee was bringing down a super sac of CPE so that it could be replaced. The super sac needed replaced due to flow issues. The material was bridged inside of the bag. When he began to lower the super sac the super sack and bar support frame used to pick up the super sac came off the hook and struck employee on the R side of head and on the R shoulder.
