What happened
As I was approaching door 26 with [REDACTED] my fork blades were tilted down & on the ground. The blades hit the dock plate & the machine came to a sudden stop. I was thrown forward & the right side of my head hit the scale module & knocked me out for a few seconds. Lump on head with a contusion. the impact also broke my glasses. I gathered myself & continued working. I did not report to a supervisor. By [REDACTED] I had a severe headache & told supervisor I was leaving because I did not feel well. I was not wearing a seat belt at the time.
