EE REPORTS INJURY TO RT AND LT EYE. REPORTS APPROACHED A RESIDENT AND ASKED HER TO COME TO THERAPY. WHILE DOING ACTIVITIES RESIDENT WAS DRINKING COFFEE OR TEA AND EE ASKED RESIDENT IF SHE WANTED TO GO TO THE GYN. RESIDENT THREW COFFEE OR TEA IN EE FACE AND THEN THREW THE MUG AT EE HEAD. EE WALKED AWAY AND LEFT THE RESIDENT WITH SUPERVISION OF OTHER STAFF.
