What happened
Annual hearing evaluation showed an OSHA recordable hearing loss in the employee 's right ear. Employee had a family emergency come up that he needed to attend to and left work for an extended period of time. Since the employee was gone we were unable to recheck his hearing to confirm the hearing loss. Therefore we are reporting this on the OSHA log even though it was never confirmed.
