What happened
Annual hearing evaluation showed an OSHA recordable hearing loss shift in the right ear. Afterwards employee had a family emergency that he had to attend to and was gone for an extended period of time. Due to his absence we were unable to get his hearing retested to verify that hearing loss occurred and was present. Due to this we are reporting the hearing loss on the OSHA log even though it was never confirmed.
