Employee was working on the East side collar. Employee had his left hand up above between the collar and the fill tube. His left hand was down underneath fill tube. While trying to secure the fill tube the auger came out of the locking shaft and it dropped into the fill tube. The fill tube then fell down and smashed his fingers in-between the fill tube and the forming collar.
