Before the incident

What happened
Upon giving a solid push to enter the plant the wheel got stuck in a crack hole broke and caused the tool box to tip as it fell to the ground. Employee stated that their hand was stuck on one side and they could not get away as it tipped trapping their hand and landing on their right toe
