What happened
On [REDACTED] employee was pulling on a sponge dragging it across the pipe to clean pipe. A second employee was on top of manhole wrapping the line on a piece of pipe to avoid the line becoming tangled. The second employee dropped the piece of pipe on the manhole and it hit the employee on top of the head.
