Unhooking chains from flask.

The employee thought the drag flask was set squarely on the ground. Unknown to him was the there was a chunk of [REDACTED] holding up the flask. The sand gave way crushing the employee 's right foot.

A flask was lowered down and rested on sand instead of the ground. EE had his foot under the flask without realizing. The sand gave way and the flask fell on top of the right foot causing a fracture
