Before the incident
Winder Operator was attempting to take off a film roll from Winder #32 after a cut-over. While attempting to take of the film roll ([REDACTED] lbs.) that was just cut-over with a [REDACTED] they noticed the film roll was not seated correctly in the feet of the [REDACTED] after beginning to lift. While the Winder Operator was lowering the roll back onto the Winder unbeknown-st to the Winder Operator the EE came around behind them to assist and tried to pull the side of the shaft to get it seated correctly. While doing this the left ring finger was pinched between the Winder Track of the Winder Arms and the film roll shaft.
What happened
Winder Operator was attempting to take off a film roll from Winder #32 after a cut-over. While attempting to take of the film roll (1967 lbs.) that was just cut-over with a [REDACTED] they noticed the film roll was not seated correctly in the feet of the [REDACTED] after beginning to lift. While the Winder Operator was lowering the roll back onto the Winder unbeknown-st to the Winder Operator the EE came around behind them to assist and tried to pull the side of the shaft to get it seated correctly. While doing this the left ring finger was pinched between the Winder Track of the Winder Arms and the film roll shaft causing a fracture & laceration to the left ring finger.
