The employee reached past two guards which resulted in thier finger being caught between a 2x4 and the second guard. The 2x4 was being pushed out from the cutter heads on the notching machine during its normal operating cycle. The employee 's finger was pushed in a downard direction between the 2x4 and the guard resulting in the crushing injury. The employee was unable to explain why they reached past the guards.
